Facts you need to know

  • Long-term care needs increase as people age. In 1999, 16 percent of all seniors received long-term care services, including one-fourth of all seniors aged 80 to 84.
  • According to the Congressional Budget Office, the number of disabled elderly persons is projected to rise by one-third from 9.2 million in 2010 to 12.3 million by 2030.
  • 44 million short term stay residents transition from a nursing home back to their home each year. A Medicare short term stay is classified as about 23 days.
  • Eighty percent of seniors have at least one chronic condition, and fifty percent have two or more.
  • The transition from hospital to home is often a critical turning point for older adults, often preceding a decline in physical and mental health.
  • Statistics show that approximately one-third of rehospitalizations can be avoided with proper transition of care from hospital to home.
  • Hospital stays have decreased in length dramatically over the years and people are being discharged with greater needs and in greater state of illness.
